Are you bringing a manual wheelchair, power wheelchair, or mobility scooter?  You will need to document if it is folding or not, if the battery type is gel or dry, the dimensions of the equipment itself.  They will also request the combined dimensions of you and the equipment - for example, when I am sitting in my power chair, how tall am I? What is the combined weight of me plus my chair?

Please note that wheelchairs and scooters must be stored inside your stateroom.  If you are considering renting a mobility device, be aware that the stateroom doors are as narrow as 23 inches wide.

If you selected an ADA room FOR ANY REASON, you will need the form, as there is a written attestation you have to sign.

Once the form is completed, return it to the email address on the form, not to us. It is required that you interface directly with the Special Access department so they can do any necessary follow up.
